Today, we will cover:
1. How to combine two crescentic osteotomy tools to form the DOUBLE CRESCENTIC cut functionality
2. How to control all the functions once they are combined.
3. An overview of the details provided on the information palette.
1. Select your first Crescentic Saw Tool: From the menu top right , select the crescentic saw tool.
2. Place the Tool: Here we will position the tool as if performing a CBLO with a 27 mm blade, exiting perpendicularly at the caudal aspect and cranially at the distal tibial crest.
Step 2: Understanding Rotation and Panning
1. Rotate and Pan:
o Rotate the fragment as usual, as far as you are comfortable to do so for secure fixation.
o Then use the Move Pivot (ACA) feature on the information palette to move the ACA to the desired location of the apex that represents the closing ostectomy of the second component of the TPA reduction. In the case of a CBLO here, it is the caudal exit point of the osteotomy
o Lock the pivot point. Now, you can rotate and also pan the fragment to your target Joint Orientation line. This is crucial for double crescentic osteotomy planning as the new position of the fragment’s osteotomy determines i) the potential position of your second crescentic osteotomy ii) the magnitude of the crescentic wedge.
o Please note- you will be able to optimise the orientation of the crescentic wedge component in your plan after the two tools are combined.
Step 3: Combining Crescentic Blades
1. Add Another Crescentic Blade: Once you have determined the wedge magnitude, you need to bring up another crescentic blade to combine with the existing one, to form the Double crescentic osteotomy tool.
o VPOP PRO automatically uses the same blade radius for both tools. Different radii cannot be combined.
2. Combining Blades:
o When the blades are close, VPOP PRO detects this and highlights them in blue.
o As they get closer, the color changes to green, indicating they are ready to combine.
o Simply release to let the blades snap together. You can unlink them at any time in the info palette or undo with ‘Control Z’
